You need your HMO ID Number, or  Health Assur ID card/ digital ID from the mobile app when visiting a provider.

Log in to your Health Assur App or contact customer service to request a hospital switch. But note that roaming services are available, so you can go to any Health Assur accredited provider without changing your primary hospital.
